Inexpensive, light weight macro rig. Just a typical dSLR with a Macro lens (Tamron 90mm F2.8 VC in this case, but any inexpensive lens will work just as fine) and an inexpensive manual flash (Yongnuo 560 III). No special mounts or fancy stuff. The big difference is the diffuser setup. A simple foam tray normally something you throw away from the grocery when you buy something that comes on it (9 x 11 here), with some paper towels to further diffuse the center where a hot spot would be. To bounce more light into the diffuser, a piece of cardboard with tin foil taped on it taped onto the head of the flash and pointed towards the diffuser. Simple!
