I have also been having the lens reflection problem. But unlike some people I do use a lens hood and a very good ND or Polarizer filter. On this specific bridge I had taken many photo using AEB, +2, 0, -2 RAW. The photos were processed in Photomatix Pro 4.1. All the photos I shot this is the only one that showed the spots. The bridge was shot at every angle possible with the sun either on my right or left shoulder, in this case with the spots it was over my right shoulder.

Can you elaborate on that? In reading what I just posted...Lens cover, polarizer filter and both were clean. Then what caused the lens flare?

you still might be getting flare from the polarizer/ND filter .....I know you said they're good but there still might be light refracting through at just the perfect angle.....really its the only thing I can think of....

This is one of those places where a ball head on the tripod can come in real handy. Even moving the orientation of the whole kit a degree or two on either axis can stop these, or make 'em worse. Then you can fix the horizon and such in post.
And again, something I learned watching the Peter Lik show this past season: holding a hat or other object up above the kit off frame will completely kill it. I've done it a few times now, who knew?
