The monitors in those pictures are designed to display real-world information about Halo 3 and enable the player to interact with them to gain access codes and unlock doors. Unfortunately the sandbox-sandbox operating systems inside these virtual computers are identical to the ones Microsoft uses, and are thus just as susceptible to system errors as an ordinary Windows box. When they had their own sandbox, virtual errors, the Blue Screen of Death popped up, clearly unknowingly to Microsoft.

Case closed.

Yup, carry-over from Halo 2 Zanzibar map. I haven't checked to see if it's on Last Resort (the Zanzibar remake) in Halo 3 as well.
