One small softbox, a Lumiquest SB-III with a speedlight directly overhead and in front of the subject, and a bare speedlight on the background.

This is actually from when I was going through the Strobist assignments early last year. This one was all about using hard light to achieve a "film noir" look.

To the best of my recollection, this was made with two lights: one gridded and directly above the glass, and another fired strategically into two white cards, back camera left and right, to provide rim reflections on the glass.
