Pretty straightforward, actually...ctrl+8 for the wide angle, and that gave me my photo (it's difficult to control the camera at this wide of an angle, btw). Then, I made another layer and made a soft circular mask around my head. Multiply mode, add noise, gaussian blur, radial blur. Then using the same mask, radial blur on the background layer. Curves to taste, and that's it.
