It's one complete lever, with one extra lever base and one of those 1x2 tiles with one connector on the top. It all fits together surprisingly snugly...

It's one complete lever, another base of a lever all attached together (in a mildly wobbly fashion), and then all attached at one end to a 2x1 with one stud on the top.
