I'm not sure what the instructions say, but here's how I see it working: turn the red handle on the big cog counter clockwise, which turns the smaller cog clockwise, which slides the plate on the left up and clicks the switch - while the arrow on the small cog goes almost a full rotation to change from "off" to "on." Reverse and repeat.

SSSidhu - there aren't any directions/instructions on the sign - I added the text in the description just now so you can read what it says. Don Solo is right: you turn the crank handle to turn the light switch on or off and it turns the indicator arrow to point at the "on" or "off". It works beautifully. It is completely charming and I want them on all of my light switches.
