The shirt is an adaptation of a pattern in an old Martha Stewart Kids or Baby magazine (I can't find which issue). You can still get the pattern online
here.

In my opinion, the instructions with the pattern are pretty terrible. For the shirt, I just adapted the pattern to be a little shorter, and added facing around the neckline and front so that I could use snaps instead of ties.
