      I used to love getting these order forms at school! I ordered many Scholastic books :).

      I don't have a full form but I do have the part that you would cut out to give to the teacher to order your choices. For some reason I must not have given it to the teacher since I still have it. Very rare for me since I always ordered something from the form. Mine is from the mid 1980's.
