Crestwood Care Centre is a skilled nursing facility specializing in dialysis, Alzheimer's care, physical rehabilitation, and surgical wound therapy. Our staff focuses on individualized patient care to bridge the gap between hospital and home for both short-term recovery and long-term care. The facility is conveniently located on the southwest side of Chicago in Crestwood, IL accessible by public transportation and major expressways.

Sending patients to multiple hospitals, clinics, and locations to accomodate complex care needs adds unnecessary stress. To minimize patient stress, Crestwood Care hires trained clinicians and partners with leading community providers to offer in-house care. Hemodialysis for patients is provided by Direct Dialysis with all physical and occupational rehabilitation therapies administered by Marianjoy Rehabilitation Center. Crestwood applies a highly-clinical, process oriented approach to determining the proper treatment for complex care solutions. Patients are assessed by clinical nurse liaisons responsible for tailoring rehabilitation, recovery, and reshaping programs. Similarly, Crestwood has a concierge staff to ensure family members of patients are comfortable and familiar with treatment programs from admission to discharge.

Crestwood is accredited by the Joint Commission - a distinction bestowed to facilities exceeding the standards of Illinois Department of Public Health. This distinction enables Crestwood to accept a variety of payment sources including: HMO Insurance plans, long-term care insurance (LTC), Medicare and Medicaid.
