Founded in 1912, Otterbein Senior Lifestyle Choices is a non-profit health and human service ministry, serving nearly 2,000 people, and is related to the East Ohio and West Ohio Conferences of the United Methodist Church.

Otterbein has five full-service retirement communities located in Lebanon, Cridersville, St. Marys, Pemberville and Lakeside; five skilled nursing and rehabilitation neighborhoods and a home health agency.

Otterbein is leading the nation in changing the face of nursing care by taking the bold step of moving elders to homes in the community at large. Otterbein Skilled Nursing & Rehab Neighborhoods offer exceptional skilled nursing care and rehabilitation in residential settings.

Neighborhoods are located in Perrysburg and Monclova in northern Ohio and Middletown and Springboro in southern Ohio.

Otterbein Home Health provides services to independent living residents, as well as individuals in the larger community.
