Generally, it's interpreted as Otoko being male. It's because male is called "otoko (男)" in Japanese. However, Otoko which Orkut Otoko defines is Otoko as Kan (漢) and Otoko as Kyo (侠). Especially, a letter (Kanji) of Kyo (侠) includes not only male but also female in the meaning.
Btw, Otoko of female has the nuance of Yamatonadeshiko (大和撫子) without limitation of Japanese female.
