If I'm using one light I carry a lightstand, SB600, umbrella, snoot, gobos and a DIY beauty dish I made out of a big bowl and a lightsphere. (Finally put that jank Gary Fong piece of S%$t to good use)
Edit: I carry it in a small Timbuktu messenger bag. I put the lightstand on the bag and then buckle the straps across it.

I'm using a Cameron air cushioned quick stand (amazing little light stand that folds up when you pick it up and opens up when you put it down), Lastolite Professional Ezybox Softbox, a fold up round reflector, and either a single Elinchrom Quadra Ranger with battery pack or a Speedlite with Pixel Knight Triggers. The lighting packs into my Lowepro backpack, light stand is usually set-up with softbox ready to go (speedlites are much faster to move around than the Quadra Rangers). It's what I used for my last location shot.

MINIMALIST? I use a Canon G11 interfit (very)cheap remote flash kit canon 430II pocket softbox and 15 cm alluminium tripod in an old jeans bag.
OR I have to ask someone to carry my other stuff while taking serious shots...

I carry it all in a small Domke bag (Domke F-5xb)
At first I wasn't sure that it would be enough. But then I made several images from it and well...

I think my minimalist setup just changed. I got a Fotodiox 8x12 softbox this weekend... small enough to handhold on the flash, and it's well made and mounts securely. So camera, SB600, Fotodiox softbox, Cactus triggers... no stand needed.

Monopod, SB600, Lumiquest SBIII. RP JrX triggers.
My complete mobile set up: Above equipment plus two Lumopro compact lightstands, LP160 flash, 43" shoot through umbrella, Photek Softlighter, gels, extra batteries and small accessories.
Everything fits in a Lumopro lightstand bag and a compact camera bag with one camera body, two zoom lenses and one flash. I place the Photek between the handles of the Lumopro bag to carry in one hand, the camera bag on my shoulder and carry a Wescott 4 in 1 set of diffusers on the other hand.

I have many common combinations, depending how minimalist I want to go.
Super minimal: X100, Cybersync triggers, one SB800, maybe a Lumiquest SBIII.
Kinda minimalist: D700, 35mm and 85mm, Cybersync triggers, two SB800s, Lumiquest SBIII, 43" compact umbrella, gels, grids, one compact stand.
Larger combinations include some or all: a D3, 20mm and 150mm, larger stands, a boom stand, another SB800, Einstein and mini vagabond, more umbrellas, reflectors and scrims, beauty dish, and foldable softbox.

What happend to the "one light theme" the OP specified? The responses read like a list of all the gear people own.
Super minimal for me would be two speedlights mounted on a single umbrella, one light stand, a reflector, and YongNuo RF-602s to trigger. Camera, lenses, gels, tape, Lumiquest LTp, filters, CF cards, etc. in a LowePro Computrekker AW backpack.

I use Lumiquest stuff because they fold up nicely into my bag. I use the LTP softbox, Softbox III and the Lumiquest Snoot. The flashes and triggers are in my bag, and I just hold the stand in my hand. It's lightweight and folds up nicely.

SB-800 or SB-700, Phottix Strato triggers (or SU-800 is I want TTL) and a heavy-duty tripod off amazon with an AS-19 plate on the top. Tripod folds down to 60cm or so and isn't too heavy, plus it'll sit evenly on any ground. It doesn't have the overall height of a regular light stand but i can live with that if I'm walking for miles to get to locations.
I shoot angling as my day job and this set-up gets me most jobs done. If I want, i can also take a 28" Apollo (when it's not too windy) or I use one of those 40cm folding softboxes, a bit like the Lastolite ones.
Other than that it's regular kit - two D2x bodies, 35mm f/1.8, 60mm macro, 70-200, 14mm f/2.8 and 17-55mm. A spare flash will generally travel with me in the car, just in case.....

It does not get any more (or less?.)..MINIMALIST than this..! The bare bones gear that is always with me in a Domke F4-AF bag consists of:
Nikon SB-400 Strobe..used on my D700 hot shoe..or more often with a
NIkon SC-28 Extension coiled TTL cord..or optically with an...
SU-4 Optical Trigger
.
Sto-fen diffuser for the SB-400....
NIkon SB-30 is my second small strobe and can be used as a slave or as an IR trigger on-camera to fire an optical slave.
Ultra-Pod II used in various ways as a strobe mount, tripod, etc
White Hankerchief..used as a diffuser, reflector or nose wipe..!
CRITICAL....Spare batteries for these units...!!!
I feel very well prepared for most of the Flashy situations that I might encounter on a day-to-day basis. This small bit of gear, coupled with the pop-up flash of the D700...and the huge ISO range of that camera gets me by 99% of the time.

Portable minimalist lighting setup?
interesting question!! :-)
When I am on holiday or on a photo walk, I normally carry my camera rucksack with me, with a little cam tripod tied to the side. Sometimes I use the tripod as lightstand (the YN 602 has a quarter inch thread, see below).
Other always-in-the-bag strobist gear (it doesnt weight that much):
- a 430 EX II with a homemade snoot ("black foamy thing")
- pack of gels
- some RF modules from Yongnuo (YN602 receivers / transmitters)
- some sheets of paper (= reflectors, diffusors)
- some model release forms (you never know ...)
- small role of gaffa tape
Mostly no lightformers (too bulky) -- normally you can bounce on something on location ... or just shoot with hard light! That's my opinion.
If I carry a lightformer whilte travelling light, it is a circular reflector or a collapsible white umbrella. First one is more versatile. Second one can be used in the rain as umbrella and can be tied to the tripod with a little gaffa tape.

It would now appear that many people posting to this thread take a stack of gear around with them when shooting! However, with the title here being about "Portable Minimalist Lighting" those of us who do actually shoot with just "one light" a la' "Zack Arias",- find it a little odd that an "Elinchrome" Quadra setup is being described as "Minimalist"! Try shooting with just one tiny strobe and a DIY cardboard "Softbox"! That's what you call a Minimalist Lighting setup!!
